License Applications

  2. Power of Attorney for both parties, legalized.

  3. Legalized business documentation of the licensee.

Assignment Applications

  1. Legalized Power of Attorney and deed of assignment.

  2. Legalized business documentation of the assignee.

  3. Details of the assignment and parties involved.

Change of Name/Address Applications

  1. Legalized Power of Attorney.

  2. Certified certificate of the change, legalized.
